FACE Coordinator Responsibility
The FACE Coordinator serves as the site point of contact and works with the FACE team to review and validate monthly program data for accuracy and completeness prior to submission. The Coordinator is responsible for ensuring all required forms and reports are submitted by established deadlines, and that staffing and contact information is current and accurate.

All electronic form confirmations will be sent to the email entered on the form. These emails will come from forms@face-program.org
Please add this to your contact list and ensure you mark the incoming messages as "not junk/spam" to ensure you continue receiving them.
If you submitted a form and did not receive an email, please reach out to FACE_DataTeam@face-program.org.
Avoid Common Mistakes: Ensure all fields are completed to the best of your ability. Incomplete forms or blank fields may delay processing and compromise the tracking accuracy.
Accuracy is Key: Take a moment to review the completed form before submission. Double-check dates of birth, participation hours, and any other details to avoid inaccuracies.
Timely Submission: Adhere to the 5th-of-the-month deadline for all monthly reports to ensure timely processing. Late submissions may affect the accuracy of program data and tracking.
